This program features real-time vital signs monitoring, lifestyle coaching, support from a bilingual care team, and coordinated care with Chinese Hospital providers—helping patients achieve better control of their blood pressure and blood sugar levels.
Unified Care’s all-in-one support system has made a significant impact on patient health and well-being. Since 2018, the program has helped thousands of patients across the country improve their health outcomes. Studies published by the American Heart Association and the American Diabetes Association reported a 75% reduction in the number of patients with poorly controlled blood sugar (HbA1c) and a 46% reduction in those with uncontrolled blood pressure. In the same period, the percentage of patients with well-controlled conditions increased by 31% for diabetes and 76% for hypertension.
