CDC-Recognized Diabetes Prevention Program
Designed for individuals at risk of developing type 2 diabetes, this program is recognized by the Centers for Disease Control and Prevention (CDC) for its evidence-based approach. Participants in the Diabetes Prevention Program learn how to make small, sustainable lifestyle changes to significantly reduce their risk of developing diabetes. Key elements of the program include:
- 16 weekly sessions followed by 6 monthly maintenance sessions.
- Sessions led by a trained lifestyle coach, covering essential skills like weight loss, increased physical activity, and effective stress management.
- Opportunities to connect with a supportive group of like-minded participants who provide encouragement and motivation throughout the program.
With Jun’s expertise, participants will receive culturally sensitive and personalized guidance to stabilize blood sugar levels, maintain a healthy weight, and adopt habits that promote long-term wellness.
Hypertension Management Program
High blood pressure is a serious condition that can lead to complications if left unmanaged. Chinese Hospital’s Hypertension Management Program is designed to support individuals struggling with uncontrolled hypertension. This program offers:
- Customized Nutrition Plans: Reduce sodium, add heart-healthy foods, and understand how diet impacts blood pressure.
- Lifestyle Coaching: Tools and resources to make sustainable changes, including stress management and regular exercise.
- Ongoing Monitoring: Continuous support to track progress and make personalized adjustments.
This program is tailored to meet each participant’s unique needs, with a focus on achieving and maintaining healthy blood pressure levels for better overall health.
