Position Summary

The outpatient registered dietitian will work with outpatients to improve their health and well- being.  The outpatient dietitian is responsible for providing quality health education to individuals at assigned site(s), the community, and staff.  The outpatient dietitian will provide Medical Nutrition Therapy (MNT) counseling to individuals and groups. This entails creating nutrition care plans using the Nutrition Care Process to specify diet plans or goals for patients with varying medical diagnosis such as diabetes, chronic renal insufficiency, metabolic syndrome, heart disease and more.  The outpatient dietitian may counsel individual patients or their families in a private or group setting. Education includes individual and group counseling, medical nutrition therapy, chronic condition self-management, and wellness.  Other responsibilities may include: nutrition research and presentations, wellness classes, creating and maintaining nutrition education materials, authoring nutrition department policies and procedures, and other responsibilities as the job and need for nutrition expertise evolves in this practice setting.

Essential Duties and Responsibilities

  • Assesses patients’ nutritional risk/health status according to Academy of Nutrition and Dietetics protocols
  • Assesses for and make referrals based on patients’ broader health, social, and emotional needs and supports based on health condition, beliefs, values, and concerns
  • In accordance with provider recommendations, partners with patient, family and/or caregivers to develop and implement a plan of care for nutrition/health education and counseling
  • Provides individual and group medical nutrition therapy, counseling and support for prevention and management of chronic health conditions
  • Works collaboratively with care team members to identify patient needs and make recommendations
  • Refers patients to resources available within the CHA system
  • Documents care plan in the medical record within 48 hours of patient encounters
  • Develops and approves nutrition protocols for patients
  • Assists the Education Department with developing, reviewing, and maintaining current instructional materials on nutrition and dietary guidance
  • Develops and assists other team members in presentation of nutrition education classes
  • Leads and facilitates classes of the organization’s Diabetes Self-Management Education (DSME) program, Diabetes Prevention Program (DPP), and Sweet Success Gestational Diabetes Management Program.
  • Assists the Integrative Health & Wellness Manager in the training and development of health
  • Assists the Integrative Health & Wellness Manager with outreach activities and integration of services
  • Attends required meetings and participates in internal and external committees as requested
  • Participates in professional development activities and
  • Follows Hospital and Department policies and procedures at all times, including but not limited to: Administrative Manual, Environment of Care, Human Resources, Infection Control, Corporate Compliance, Code of Ethics, etc.
  • Attend, actively participate and complete in-services, training classes, mandatory classes, seminars/workshops, staff meetings, exercises and drills; reads all department communication and assigned materials.
  • Customer Service – Provides excellent customer service and shows compassion to all patients, visitors and co-workers. Seeks feedback to ensure all needs are met. Anticipates and recognizes the concerns of others, even if those concerns are not openly expressed.

Qualifications

  • Bachelor’s Degree in Dietetics or related field, and current certification as a Registered Dietitian in the state of California, and evidence of continuing professional education to maintain registration.
  • BLS – Basic Life Support Certification – issued by AHA-American Heart Association.
  • Bilingual in Cantonese and English is highly preferred.
  • Give presentations and speak publicly regarding nutrition/health topics
  • Skill in establishing and maintaining effective working relationships with employees and the public
  • Knowledge of evidence-based medical nutrition therapy and educational, and dietary principles to direct high quality nutritional care (i.e. motivational interviewing techniques, weight management, diabetes management, nutrition, and preventive health & wellness).
  • Knowledge of diet and nutritional assistance programs
  • Ability to track information and prepare reports
  • Ability to identify problems, provides information, and recommend solutions
  • Ability to interpret, adapts, and applies appropriate written guidelines and work practices
  • Ability to work on a computer with a basic level of proficiency, including Microsoft Office Suite applications
  • Familiarity with electronic charting
  • Strong communication skills.
  • Ability to effectively present information, both verbal and written.
  • Ability to take initiative, adapt to changing priorities, and work independently
  • Strong time management and prioritization skills
  • Ability to multi-task with a high level of efficiency and attention to detail

Physical Requirements

While performing the duties of this job, staff is regularly required to sit, stand, walk, talk and/or listen. He/she uses his/her hands to do computer work, write reports, do equipment set-up/cleaning/storage, clerical support, etc. He/she will be using the phone frequently. Good vision is needed to be able to read schedules, enter accurate data, etc. He/she must have good general health and demonstrate emotional stability so as to carry out the above-enumerated duties.

  • Use proper body mechanics when handling equipment
  • Standing, walking and moving 50% of the day

Complies with Chinese Hospital Compliance Handbook including Code of Ethics and all statutes, regulations, guidelines applicable to federal and state programs. Responsibilities include, following the guidelines and reporting suspected violations of any statute, regulations, agreements or guidelines applicable to all healthcare programs.
